for annual boosters, however the Dept and the University of Florida
are recommending boosters every FOUR MONTHS. Eastern Equine Encephalitis
protection is not good for more than ONE YEAR.

MEDICAL INFORMATION FORM
The USA Equestrian Safety Committee is introducing
a rider Medical Information Form sponsored by Troxel Helmets, with the
recommendation that each rider/driver/handler complete a copy of the form
and place it in an outside coat, shirt or pants pocket whenever competing
at all recognized competitions. The committee advises that the information
exhibitors include in the form will assist medical personnel in the event
of a serious accident or injury. Please inform your trainer and/or groom
of the location of your completed form and be careful if borrowing another
exhibitors jacket.
Forms are being mailed to competitions with the request that they be made
available to exhibitors in the show office. Forms are also available as
downloads on our website under Safety and under Competition downloads
and will be mailed to all members with their 2004 membership cards

by Dr. Leroy Coffman, Florida State Veterinarian. This will greatly facilitate
our movement inside the state, and applies for the State of Florida.
